Job summary

Part-time Social Worker
Shift: Monday - Thursday, 8am - 4pm
Location: 4108 Norfolk Street, Burnaby, B.C., V5G 0B4
Salary: $42 per hour

Social Worker Must Haves
  1. Graduation from a Bachelor's of Social Work Program.
  2. Registration with the BC Association of Social Workers.
  3. Satisfactory proof of COVID-19 vaccinations.
  4. Passes and complies with the Ministry of Justice Criminal Record Check upon hire and every 5 years thereafter.
  5. Ability to effectively communicate in English, both verbal and written.
Social Worker Nice to Haves
  1. 2 years experience in a social worker capacity.
  2. Gerontological / Senior's care environment experience.
  3. Maintains First Aid and CPR certification.
  4. Proven experience working unsupervised and providing direct supervision.
Responsibilities
  1. Interview prospective residents and their families to determine care needs and provide appropriate counselling to ensure a smooth transition.
  2. Liaise with outside agencies and health authorities to facilitate planning and scheduling of resident admissions, discharges, and transfers.
  3. Plan, schedule, and facilitate resident admissions, transfers, and discharges to maximize occupancy and resident/family satisfaction.
  4. Provide counselling and support to residents and families experiencing transitional distress.
  5. Plan, develop, organize, and facilitate resident/family supports through contact with community health agencies and support organizations.
  6. Participate in care conferences and multidisciplinary team meetings.
  7. Maintain appropriate clinical records, statistics, and resident health documentation.
  8. Collaborate with the care manager to provide guidance to staff, promote new social work initiatives, and develop policies & procedures.
  9. Develop, implement, monitor, and evaluate social work service programs and quality improvement initiatives.
  10. Participate in site leadership discussions related to health & safety, resident care, and operations.
