Job summary

Durham Region’s Lakeview Manor in Beaverton, Ontario seeks a Social Worker to provide counselling, education and support for residents, families and staff within Innovative Person-Centered Care.

You will respond to inquiries, coordinate transfers, complete assessments, engage with community services and participate in CQI and ethics committees, while maintaining confidentiality and working with interprofessional teams.

Qualifications

  • University degree in Social Sciences (Psychology, Sociology or Social Work) or equivalent; Master's in Social Work preferred.
  • Current registration with the Ontario College of Social Workers and Social Services Workers as a Social Worker (SW).
  • Formal training in gerontology; knowledge of aging, disease, death and dying.
  • Comprehensive knowledge of the Fixing Long Term Care (FLTC) Act and related policies.

Responsibilities

  • Encourage and support an Innovative Person-Centered Care (IPPC) environment.
  • Respond to inquiries from all stakeholders about the Home's programs and services and act as a referral source and liaison to those seeking information pertaining to long term care, resident care, aging and palliative care.
  • Coordinate and support internal transfers in conjunction with appropriate legislation.
  • Provide supportive counselling services and coordinate problem solving meetings with residents, families and other stakeholders as deemed necessary.
  • Complete resident post-admission psycho-social assessments to meet MLTC regulations.
  • Engage with various community-based agencies or programs as deemed necessary.
  • Participate in internal committees and meetings such as Palliative Care, Ethics, P.I.E.C.E.S and Family Support Group.
  • Act as a resource to staff and management, and develop and deliver education on regional and home specific policies and procedures, including problem solving, communication, ethics and abuse-free care.
  • Schedule and attend care conferences as required.
  • Participate in CQI initiatives and Accreditation process.
  • Work with interprofessional teams to coordinate resident-specific services, such as memorial services and final Farewell.
  • Be available to work weekends and evenings, as needed.

Lakeview Manor

This position is located at Lakeview Manor in Beaverton, Ontario.

Social Worker

Reporting to the Director of Care, the Social Worker is responsible for providing support, counselling and education to residents, family members, staff and the community at large to create a harmonious living and working environment, under the umbrella of Innovative Person-Centered Care.

Specifically, the incumbent will:

  • Encourage and support an Innovative Person-Centered Care (IPPC) environment

The successful applicant will possess:

  • A University Degree in Social Sciences (Psychology, Sociology or Social Work) or equivalent; a Master's Degree in Social Work is preferred
  • Current registration with the Ontario College of Social Workers and Social Services Workers as a Social Worker (SW)
  • Several years relevant experience in a health care setting, preferably working in a residential setting such as a long-term care facility
  • Formal training in gerontology, along with knowledge of aging, disease, death and dying
  • Comprehensive knowledge of the Fixing Long Term Care (FLTC) Act and other relevant policies, procedures and legislation
  • Demonstrated problem solving, conflict resolution, mediation, negotiationand counselling skills
  • Excellent written and verbal communication skills, coupled with strong facilitation and presentation abilities
  • Working knowledge of MS Office, including Word, Excel and PowerPoint; experience with Electronic health care record (eHCR)
  • Extensive knowledge in palliative care and the use of ethical frameworks when solving problems
  • P.I.E.C.E.S certification preferred
  • Proven organizational, time management and project management skills
  • The ability to work collaboratively with residents, family members and members of the interdisciplinary health care team
  • Ability to maintain confidentiality and unconditional positive regard, and empathetic ability to develop therapeutic relationships
Management & Exempt Salary Grade 4
  • Salary: $90,818 to $113,523 per annum
Conditions of Employment

All applicants are expected to comply with the Region of Durham's Code of Ethics and Code of Conduct Policies throughout the recruitment process. The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within six (6) months from date of hire. Additionally, tuberculosis screening will be required in accordance with the FLTCAct (O.Reg. 246/22). Proof of education, qualifications and any other job bona fide requirements will also be collected. Finally, compliance with the Mask Fit Testing Program is required and will be coordinated as part of the orientation process.
