Qualifications

Health Services - Long Term Care

Status: 1 Contract Full-time (15 month term)

Salary Range: $92,153 - $115,189 per annum plus comprehensive benefits

Location:Tall Pines - 1001 Peter Robertson Blvd, Brampton

Hours of work:35 hours per week; core business hours are 8:30am-4:30pm, Monday-Friday. Some flexibility is required, including participation in a rotational weekend on-call schedule for managers, as well as potential evening availability for events and meetings

What you will do in this role:

  • Review applications, assess admission suitability, coordinate placements with Ontario Health atHome, and manage internal transfers for appropriate care accommodations
  • Analyze information and provide recommendations for behavior and dementia care, and interpret relevant legislation (e.g., Ontario Health Care Consent Act, Advance Care Planning)
  • Lead the Palliative Care program, co-chair the Pain/Palliative Care/Ethics Committee, and provide counseling and support regarding care planning, crisis intervention, palliation, and bereavement
  • Complete documentation, analyze trends, and provide recommendations based on case findings and risk assessments
  • Guide residents, families, and staff on treatment plans, legal matters, and policies. Supervise student placements, assist with staff training, and coordinate resident-focused events (e.g., Seniors' Month, Family Council Week)
  • Build relationships with internal/external stakeholders, ensure compliance with standards, and participate in multi-disciplinary decision-making processes.
  • Contribute to management decisions, provide weekend supervision, and maintain communication with the Administrator and Management Team

What the role requires:

  • A Bachelor of Social Work degree from a recognized university and membership in good standing with the Ontario College of Social Workers and Social Service Workers
  • Minimum 2 years’ experience in long-term care environment
  • Counselling experience in a long-term care centre or social service agency that serves the elderly.
  • Knowledgeable of the psychosocial needs of the long-term care population
  • In-depth knowledge of available services/programs to assist clients and their families
  • Develop strategic relationships in order to navigate health and social service systems
  • Knowledge of gerontology, Alzheimer disease and related dementias
  • Knowledge of relevant Regional and Provincial legislation including the Fixing Long Term Care Homes Act (FLTCA), Health Care Consent Act, Substitute Decisions Act, MFIPPA, PHIPA policies and procedures and other provincial acts
  • Current knowledge related to the Consent and Capacity Board and Powers of Attorney
  • Prior community development/ work experience in community services with the elderly population

Skills/Abilities:

  • Demonstrated skills in assessment, facilitation and counselling
  • Strong communication skills both written and verbal to be able to effectively work with residents, families, staff, volunteers and all internal and external stakeholders
  • Effective conflict resolution skills to assist residents and family members
  • Leadership skills to motivate; and to provide coverage in the absence of the other management team members including weekend supervision
  • Public speaking skills to promote long-term care centres
  • Strong problem-solving skills to be innovative and creative in developing strategies that successfully meet the needs of residents and families
  • Strong organizational/planning and time management skills
  • Interpersonal skills to work effectively with residents, staff, volunteers and students
  • Budgeting skills to assist in formulating and administering budget and record details of expenditures such as Family Council funds, resident purchases and special events
