Are you a compassionate, detail-oriented Social Work Assistant ready to make a difference? At St. Paul’s Hospital, we are seeking a skilled professional to join our Renal Program team, supporting patients across a variety of clinics and program areas. This role involves providing services both in person and virtually, working closely with Social Workers and other healthcare professionals to ensure patients receive the support they need.

As a Social Work Assistant, you will

  • Assist clients and families in navigating external resources, including community services, benefits, and housing.
  • Offer practical support with transportation, finances, and other needs related to their care journey.
  • Escort patients to important community appointments and coordinate logistics to support safe care transitions.
  • Keep accurate, up-to-date information on community agencies, services, and resources, ensuring patients have access to relevant supports.
  • Help facilitate smooth admission and discharge planning by coordinating with care teams and external partners.

Education Completion of a recognized Social Service Worker or Social Service Diploma Program, or an equivalent combination of education, training, and experience.

Your Day to Day

  • Collaborates with Social Workers on delegated tasks from psychosocial assessments.
  • Provides follow-up psychosocial support during patient admissions and discharges, including sharing resources, completing forms, and arranging transportation.
  • Helps patients apply for housing, income, transportation, and community supports by providing information and ensuring applications are completed.
  • Assists with practical tasks like filling out forms, writing letters, obtaining consents, arranging transportation, and mailing applications.
  • Maintains and updates supplies of forms and resource information on community services.
  • Develops resource materials for patients and families on services and supports.
  • Supports patient goal-setting and helps resolve issues in a collaborative manner.
  • Completes reports and updates records on assistance provided.
  • Provides guidance to practicum students.
  • Follows health and safety procedures.
  • Performs other related duties as needed.
