Job summary

Bayshore HealthCare in Oshawa is seeking a dedicated social service professional to support three transitional care units. You will execute care plans, provide in-person and virtual support to patients and families, coordinate referrals, and assist with discharge planning under supervision of a Licensed Social Worker.

Ideal candidates will hold a Diploma in Social Service Work, be in good standing with the Ontario College, have computer skills, and a valid driver’s license for community visits.

Qualifications

• Minimum of two years of community‑based therapy experience

Responsibilities

• Execute the social and wellness care plan under the direct supervision of a Licensed Social Worker.
• Provide in‑person and virtual support to patients and families, including wellness checks, food and housing securities and social community support connections.
• Engage with community support services to follow up on referrals made by the social worker, ensuring referrals are actioned and a follow‑up date is established.
• Arrange and manage transportation services for patients to community appointments, and at times accompany them.
• Maintain and manage a caseload efficiently, ensuring priority code timeframes are met.
• Utilise a client‑centred approach to identify, prioritise, and monitor potential psychosocial problems and goals.
• Provide education and educational resources that best meet patient needs to support specific conditions or social concerns.
• Advise clients and caregivers on available community resources and facilitate referrals when appropriate.
• Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
• Comply with continuous quality improvement initiatives.
• Adhere to Bayshore Policies and Procedures.
• Participate in quality activities and continuous improvement initiatives in keeping with the company’s Quality Management System.
• Participate in proactive health and safety activities.
• Maintain confidentiality of client and corporate information.
• Complete other tasks as requested.
• Participate in ongoing internal and external continuing education activities.
• Document and report all tasks and information according to Bayshore policy.
• Maintain treatment records in accordance with the Standards of Practice of the Ontario College of Certified Social Workers and Social Service Workers.
• Demonstrate agility in decision‑making and think critically to respond to a rapidly changing environment.
• Maintain positive and collaborative working relationships within the interdisciplinary health care team.

Role Summary

This position covers three transitional care units in Oshawa and Pickering.
