Position Functions

The Social Service Coordinator will have primary responsibility for engaging with residents and their families in the facility, serving as both a counselor and case manager. Their duties include assist in conducting psychosocial assessments, providing counseling, coordinating admissions and discharges, and organizing support groups and educational programs for residents and families. Additionally, the Social Service Coordinator will oversee the coordination of the Resident Council and Family Council and collaborate with the Activation Supervisor to develop and support volunteer and pastoral services.

The Social Service Coordinator will also work with Ontario Health Home to establish and manage the waiting list of this home and coordinate the admission of residents to fill the beds in accordance with the Pre-Occupancy Plan approved by the Ministry of Long-Term Care. In collaboration with the Administrator and management team, the Social Service Coordinator will submit the Pre-Occupancy Plan to the Ministry of Long-Term Care and participate in the Pre-Occupancy inspection. Also, the Social Service Coordinator will provide support to new residents and their families during the admission process.

Responsibilities

Responsibilities shall include but are not limited to the following:

  • Establish objectives for, organize and administer the Social Services Department;
  • Develop and implement Policy and Procedure for Social Services Department;
  • Prepare statistical reports and other reports relating to Social Services;
  • Complete the social, emotional assessment and apply tests for cognition of all residents;
  • Assist in creating suitable physical, social and spiritual environments for the residents and help residents with environmental difficulties which interfere with their maximum enjoyment of benefits as residents of the Home;
  • Help the residents adjust to the new life in the facility on admission;
  • Provide advocacy services where the resident/ family is unable to do so in the role of client representative;
  • Interview applicants, residents, and their relatives to obtain social histories, coordinate liaisons with Local Health Integration Network (LHIN) placement services and write minutes and reports for Admissions and Discharge Committee;
  • Maintain liaison between Home, residents, relatives and the community and provide counselling services and crisis intervention;
  • Chair in Interdisciplinary Committee meetings and to take follow-up actions as appropriate;
  • Organize short-stay programs (pre & post admission, co-ordination of respite, supportive, emergency services);
  • Support the Activation Supervisor to plan in programs, parties, outings, activities, etc. with the residents/ representative in accordance with the objectives of the Social Services Department;
  • Help and assist the Resident Council and Family Council;
  • Assist residents in securing appliances and equipment as required, such as orthopedic, hearing, dental or optical aides, or wheelchairs;
  • Help residents in applying for appropriate subsidies and grants, e.g. I.P.P., O.A.S., G. I.S., GAINS, C.P.P., F.B.A., etc. and assist them in financial planning;
  • Collect and provide residents with information about community health and welfare resources;
  • Make arrangement for post-discharge cases of residents, assess the adequacy of the Home Care facilities and assist residents and relative in accepting alternative accommodation replacement;
  • Receive visitors and provide tours for visitors;
  • Maintain liaison between the Home and the community agencies, particularly to foster close relations between the Home and the greater Chinese Community in Ontario;
  • Assist with the recruitment, the orientation, training, supervision and recognition of volunteers;
  • Conduct monthly audit for social services;
  • Coordinate with the Activation Supervisor to provide spiritual and religious support to residents, family and staff as required;
  • Work with the Director of Resident Care to provide support to staff, residents and family regarding program of Palliative Care;
  • Provide information and support to staff and family members (caregiver) to promote quality care to residents to meet their needs;
  • Assist the Administrator in planning, developing and managing community services and outreach programs;
  • Co-ordinate and chair the Ethics Committee meetings;
  • Co-ordinate and organize the newsletter publishing both internal and external;
  • Conduct services survey to residents and family and compile report to each department for follow up;
  • Coordinate the investigation of complaints and concerns from resident/ family and file report;
  • Undertake any other duties as assigned by the Administrator.

Qualifications

  • A degree in social work, preferably MSW; current membership of; Ontario College of Social Workers and Social Service Workers (OCSWSSW) in good standing.
  • Knowledge of gerontology, experience on Long-Term Care setting preferred.
  • Excellent interpersonal communication, organizational skills and human skills.
  • Demonstrated family, group and crisis intervention counseling skills.
  • Strong team work, consultation skills.
  • Ability to work as a member in an interdisciplinary team.
  • Proficiency in both English and Chinese.
