The Social Worker / Social Services Coordinator reports to the Executive Director and is expected to provide orientation, informal education, support, and counseling to residents, families, and friends to promote a sense of comfort and well-being, as well as overall empowerment among individuals.
Responsibilities :
- Develop, implement, and evaluate Social Service Programs
- Provide support to department managers in team member intervention when needed.
- Facilitate support groups for families and/or residents.
- Provide bereavement counseling for residents, families, and team members as needed.
- Display awareness and evidence daily of why feelings matter most to individuals.
- Coordinate Family Council meetings.
- Act as the Home Resident Placement Coordinator in the absence of the Admissions Coordinator.
- Participate in tours of the Home with potential applicants and/or their representatives.
- Maintain a waitlist in conjunction with CCAC.
- Maintain communication with placement agencies, applicants, and representatives to ensure occupancy remains above LHIN-specific targets.
- Conduct outreach services by maintaining contacts with hospital social workers, discharge planners, community agencies, physician offices, and other referral sources.
- Understand and participate in the improvement of quality indicators, including resident satisfaction surveys and action plans.
- Perform other tasks as assigned.
Qualifications :
- Diploma in social services, gerontology, or a degree in Social Work.
- Registered under the Social Work and Social Services Work Act with a current general certificate of registration.
- Or relevant experience as a Social Services Coordinator or Worker in a LTCH, with specific experience requirements based on employment history.
- Previous experience in a geriatric social-related field is an asset.
- Strong interpersonal skills and ability to interact courteously with residents, the public, employees, and volunteers.
- Excellent written and verbal communication skills.
- Ability to maximize positive social interactions efficiently.
- Good organizational skills.
- Knowledge of RAI-MDS; AIS certification preferred, or must be completed within one year of hire.
- Second language is an asset.
- Demonstrate professional values such as respect, autonomy, and protection of individuals' rights.
- Ability to listen and respect others' points of view.
- Clear vulnerable sector screening, TB test results within 6 months, and supervisory references.
