Job description

The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.

The North Lambton Community Health Centre is currently seeking applicants for the following position:

Full Time Permanent Social Service Worker

Harm Reduction and Hepatitis C Care Team

37.5 Hours/Week

The Social Service Worker provides emotional, psychological, and social health support to people infected with or at risk of contracting Hepatitis C. Working with a care team focused on the needs of people significantly marginalized by poverty, addiction, mental health and housing issues, this SSW will provide a range of interventions to assist people living with Hepatitis C to cope with their chronic illness, social difficulties and side effects of treatment. This will include street outreach services, counselling, psycho-social assessments, community services, social support programs, as well as educational services for staff and/or volunteers, as required.

Qualifications:

  • Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW);
  • Social Service Worker diploma;
  • Minimum three years’ experience in a community setting; experience in a primary care setting is an asset;
  • Comprehensive understanding of the social determinants of health and harm reduction principals;
  • Experience in case management and establishing service linkages both internally and with external agencies;
  • Commitment to working with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding;
  • Ability to maintain complete and accurate records in accordance with the Centre’s policies;
  • Strong computer skills, including the use of MS Office products and Electronic Medical Records.

This position enjoys a supportive environment with flexible hours, 37.5 hours per week Monday to Friday. The rate of pay for this full-time permanent position is $52,931 - $62,865 (on a six-step grid). The rate will be based on experience and education. We are proud to offer medical benefits, the Healthcare of Ontario Pension Plan, and paid vacation to our valued employees. The incumbent will primarily be required to work at our Forest site and outreach locations.
