The salary range for this position is CAD $49.20 - $67.08 per hour.

Detailed Overview

The Site Access Coordinator is responsible for managing the effective utilization of hospital resources by coordinating patient placement and bed utilization on site and at designated Fraser Health acute sites in collaboration with the Patient Transfer Network (PTN). The role involves implementing policies, providing guidance to healthcare teams regarding patient access and community transitions, and managing after-hours patient services and staffing.

Responsibilities
  • Facilitates timely patient placement at acute and designated FH sites, assessing capacity, prioritizing issues, and leading daily bed meetings.
  • Coordinates with PTN and community resources to identify appropriate care, develop contacts, and discuss bed availability.
  • Plans transfers, including arranging patient and equipment transfers, and reports potential discharges.
  • Advises FH leadership and staff on policies, assesses critical situations, and recommends best practices.
  • Supervises clinical staff during after-hours, providing work direction and resource management.
  • Acts as a clinical resource, guiding staff and assisting in resource allocation and problem-solving.
  • Develops and recommends policy and procedural changes to optimize patient access and flow.
  • Analyzes statistics and trends to manage patient populations effectively.
  • Investigates incidents and participates in quality improvement activities.
  • Communicates with physicians, families, and departments regarding nursing practice and policies.
  • Monitors nursing practice quality and safety, identifying improvement opportunities.
  • Participates in internal committees and communicates issues and opportunities for improvement.
  • Notifies management of safety, legal, or media issues.
  • Initiates emergency preparedness actions and manages tissue and organ donation requests.
  • Performs other related duties as required.
Qualifications
Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Five (5) years' recent clinical experience in an acute care setting, including nursing leadership, with preference for experience in critical care or access and planning.
