Job Description

PLEA Community Services is currently seeking a Temporary Full-Time Services Coordinator for our U-Turn / U-Link team located in Surrey, BC. This temporary contract is proposed to start on October 20, 2025, and end upon the return of the incumbent. This position requires BCGEU membership.

The Services Coordinator monitors and upholds the quality of services provided to youth in a family caregiver home. These youth may require specialized needs related to addiction, mental health, and challenging behaviours. The employee will coordinate all aspects of participant care: overseeing family caregiver homes where the youth reside, leading support workers, and assessing the progress of service delivery plans.

Hours of Work :

The usual hours of work are Monday to Friday, based on a flextime scheduling arrangement, including days and evenings, totaling 35 hours per week.

Education / Experience Requirements :

  • A Bachelor’s degree in child and youth care, social work, criminology, or psychology;
  • Two (2) years recent related experience working with vulnerable children and youth involved in youth justice, mental health, medical services, or other community services, and their families / Family Caregivers; and
  • Experience working in residential, community care, licensed, or foster home settings.

What You Will Do :

  • Provide orientation and training to new Family Caregivers regarding program expectations, participant circumstances, and care team responsibilities.
  • Supervise and maintain regular contact with Family Caregivers, problem-solve issues related to family functioning, and use techniques such as active listening, conflict resolution, and de-escalation.
  • Conduct in-home visits, perform risk assessments, and ensure compliance with health and safety standards, contract obligations, and policies. Develop and follow up on strategies to address issues.
  • Ensure caregiving and respite files are current, accurate, and complete; address record-keeping issues as needed.
  • Coordinate all aspects of participant care from intake to discharge, ensuring relevant parties have necessary information.
  • Conduct comprehensive assessments, make referrals, and lead development and review of service, safety, and risk management plans.
  • Represent the program in case management and meetings, reporting on progress, addressing issues, and adjusting services as needed.
  • Complete required reports, ensure documentation is current and compliant.
  • Respond to critical incidents and emergencies, recognizing crises, implementing strategies, and complying with reporting responsibilities.
  • Verify Family Caregivers' pay information and review expenses for reimbursement.
  • Uphold confidentiality and perform other related duties as required.

Minimum Requirements :

  • Satisfactory Criminal Record Clearance through the Ministry of Children and Family Development.
  • Occupational First Aid Level 1 certification or willingness to obtain prior to hire.
  • Use of personal vehicle for work-related travel within Lower Mainland.
  • Valid Class 5 BC Driver's License and suitable driving record.
  • Legal authorization to work in Canada.
