Job Description

As a Service Coordinator, you will ensure our clients receive timely, seamless care. Your communication skills will be vital in coordinating services with clients, Intake Specialists, and the Customer Service team. Work in a dynamic environment where multitasking and problem-solving are key. Demonstrate enthusiasm and empathy, especially in sensitive conversations, and manage relationships effectively. This role offers the flexibility to work from home and the satisfaction of making a positive impact on people's lives.

Salary: Commensurate with experience

Hours of Work: 34 hours/week. Hybrid role with shifts on Tuesday to Saturday, including mornings and evenings.

Reports to: Supervisor, Client Services

Responsibilities

Coordination and Scheduling

  • Coordinate and schedule client service plans within assigned geographic areas.
  • Review and optimize PSW schedules to improve efficiency.
  • Collaborate with team members to ensure excellent service and manage expectations.
  • Develop and update service delivery schedules based on client needs.
  • Maintain documentation to support service provision.

Customer Service

  • Build trust and rapport with staff, clients, and families to meet their needs and resolve concerns.
  • Provide accurate information courteously and professionally.
  • Troubleshoot issues, handle difficult situations, and escalate when necessary.
  • Respond promptly to inquiries via phone, email, or written communication.

Information Management and Reporting

  • Complete relevant paperwork accurately and timely.
  • Document information in electronic records following policies and legislation.
  • Keep client and stakeholder information current and properly documented.
  • Notify stakeholders as required and respond to communication modes appropriately.

Relationship Management

  • Participate actively in the Client Services Team, supporting colleagues and achieving departmental goals.
  • Collaborate with external partners such as LHIN, government agencies, and suppliers.
  • Provide excellent customer support internally and externally.
  • Model organizational values through personal behavior.

Qualifications

  • Post-secondary education in a health-related field preferred.
  • At least one year of experience in community health care service coordination.
  • Additional language skills are advantageous.
  • Extensive knowledge of community resources and supports.
  • Excellent communication, organizational, and time management skills.
  • Ability to work efficiently in a high-paced, stressful environment.
  • Good judgment in assessing difficult situations.
  • Ability to work independently on routine tasks.
  • Knowledge of office practices, procedures, standards, and medical terminology is preferred.
  • Proficiency in Microsoft Office is preferred.
