Role Overview

The Service Coordinator coordinates staff scheduling with families for Community Respite and private Home Care support.

Key Responsibilities
  • Provide leadership through coaching and communication with families, clients, staff, and stakeholders to meet their needs effectively and promptly.
  • Ensure honest, open, timely, and accurate communication to keep team members informed.
  • Deliver exceptional customer service with professionalism to clients, staff, external agencies, and partners.
  • Support administrative systems and processes for the Comprehensive Care team.
  • Coordinate scheduling of support services, monitor, and adjust schedules daily to ensure compliance and coverage.
  • Maintain accurate records for employees, clients, and families.
  • Respond promptly to high volumes of calls, emails, and requests.
  • Stay informed about community resources and referral options.
  • Schedule mandatory training for employees as needed.
  • Ensure all paperwork is completed accurately and on time.
  • Have thorough knowledge of DDRC Home Based Services programs.
  • Review and track timesheets, mileage, and expense claims; submit for approval.
  • Prepare weekly error-visit documentation.
  • Perform additional duties as assigned by the Team Leader, Comprehensive Care.
What You Bring To The Role
  • Minimum 2 years post-secondary education or a combination of 1 year post-secondary education and experience.
  • Ensure all mandatory certifications and training are current.
  • Attend required meetings and planning sessions; seek information if absent.
  • Participate in agency events, fairs, and transition seminars.
  • Remain calm and professional in stressful situations.
  • Pay close attention to detail under tight deadlines.
  • Proficient in MS Office, especially Excel, Word, and PowerPoint.
  • Knowledge of relevant legislation and professional standards.
  • Experience with Creating Excellence Together (CET) Accreditation is an asset.
