We are searching for a Full-time Seniors Navigator to support the Nursing Home Without Walls project in Moncton, New Brunswick.

This is a temporary contract for one year with an opportunity for renewal.

The Nursing Home Without Walls team will be based out of Pavillon Richard and Pavillon Landry, located in Moncton across from the University of Moncton.

About the Opportunity

This is a Nursing Home Without Walls Initiative

Support the development and execution of community outreach programs for the older population to meet the objectives of the project. The community includes Moncton, Riverview, Dieppe, and surrounding areas.

  • Ensure seniors have access to appropriate services and information related to aging in place
  • Provide social health initiatives to counter social isolation and loneliness among seniors and their caregivers
  • Increase knowledge on health-related issues important to aging in place and healthy aging for seniors and caregivers
  • Empower the local community to respond to the needs of an aging population

Support the evaluation of how social and built environments can foster healthy aging by creating and implementing evaluation tools and reports on initiatives carried out.

  • Based on the NB Home First Strategy and the NB Strategy on Aging/Family Plan, provide services and support options needed for aging in place
  • Develop programs that address social isolation
  • Provide health promotion opportunities
  • Create a supportive community environment for seniors and caregivers
  • Disseminate information supporting aging in place

Examine the capacity of community-level approaches to address health inequalities.

  • Support interventions for aging in place
  • Build capacity of social contacts so vulnerable seniors can remain safely at home
  • Provide opportunities for community members to collaborate with the nursing home to support seniors
  • Contribute to determining best practices for aging in place using existing community infrastructure
  • Advocate for evidence-informed best practices

  • Promoting independence and choices for seniors

Business hours are typically 9:00-5:00; some weekends and evenings may be required.

About You

In addition to valuing continuous improvement, collaboration, and accountability, you bring:

  • College or university education in social work, gerontology, healthcare, project management, business management, or related fields
  • Verbal and written communication skills in English and French (required)
  • Knowledge of aging, age-friendly communities, resources, and issues related to older populations
  • Understanding of performance measurement and evaluation
  • Understanding of project management
  • Excellent self-motivation and problem-solving skills
  • Strong communication skills and ability to build positive relations and collaborations
  • Experience engaging people through assessments or group work is an asset
