Seniors Community Support Worker

Mission Community Services Society is dedicated to helping people, changing lives, and building community. With a legacy dating back to 1972, MCSS has continuously strived to support community members in need.

The purpose of this position is to assist in the Senior’s Connection, Meals on Wheels and Better at Home programs that provide services to the Senior population in Mission. The Seniors Community Support Worker will help in identifying client problems, needs and risks, develop and implement short-term, issue-specific intervention plans within program guidelines, and assist with day-to-day operations within the Seniors programming.

What you’d be doing:

  • Gather information relevant to the client’s problems, needs and risks by interviewing, observing behaviour, meeting with caregivers and service providers and using a variety of inventories, checklists, and questionnaires.
  • Review information gathered to identify problems, needs and risks.
  • Develop and implement short-term plans in consultation with the program coordinator.
  • Support seniors through helping them navigate through the process of connecting to services and resources that helps aid them in their day-to-day life. This will be done in one-one-one settings, group settings and through educational sessions.
  • Evaluate the effectiveness of the intervention plan, report on clients’ progress, and inform program coordinator.
  • Reach out to isolated and vulnerable low-income seniors in our community, including rural areas, through outreach, referrals, as well as a marketing campaign.
  • Outline services provided by the program and/or organization. Provide information on and referrals to other community service providers, resources and professionals as required.
  • Maintain related records and statistics and provide reports to the program coordinator as required.
  • Liaise with and/or promote the interests of clients with other community service providers, professionals and school personnel as required.
  • Accompany clients to meetings and appointments as required.
  • Conduct thorough intake of clients and assist with getting them in touch with the various resources.
  • Field various phone calls from clients, and drivers, including making friendly calls and home visits.
  • Assist with the completion government forms, rental assistance applications, tax returns and more, while helping to assist with filing.
  • Make connections with Seniors at the Mission Seniors Activity Centre.
  • Prepare invoicing for various program needs.
  • Assist with delivery of frozen meals for Meals on Wheels when necessary.
  • Perform other related duties as required.

What you bring:

  • Post-secondary education, minimum one (1) years’ experience working with relevant population or equivalent combination of education and experience.
  • Excellent communication, organizational, computer and problem-solving skills essential.
  • Culturally competent.
  • Knowledge of local resources.
  • Successful completion of Criminal Record check.
  • Class 5 driver’s license and a reliable vehicle.

Wage: Grid 11 ($27.54-$31.61)
