Job description

As an integral member of our innovative interdisciplinary Primary Care Support Team, the Senior Social Worker will act as a subject matter expert and support the Department Manager in providing overall direction and ongoing development of the program. They will maintain work systems, procedures, and policies that enable and encourage the optimum performance of day-to-day clinical coordination of services.

The Senior Social Worker will actively collaborate on healthcare initiatives, utilizing evidence-based practices to support patients and families facing mental illness and other challenging life situations. They will serve as a team resource and role model to promote positive working relationships and effective team building. Responsibilities include providing consultation, clinical assessments, counseling, education, and advocacy in both individual and group settings, with the overarching goal of improving and optimizing mental health and wellness.

Required Qualifications

Education

  • Master’s Degree in Social Work
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers

  • Excellent psychosocial assessment skills
  • Advanced knowledge and application of social work values, ethics, theory, and clinical practices in an outpatient setting
  • Proficiency in evidence-based therapeutic modalities, including CBT and other solution-focused approaches
  • Strong competence in the classification and treatment of psychiatric illnesses such as anxiety, affective disorders, OCD, personality disorders, and depression
  • Experience in planning and delivering social work interventions, both individually and in group settings, with patients, families, caregivers, interdisciplinary teams, and community resources
  • Superior ability to respond to situations using advanced critical thinking skills and a calm, therapeutic approach
  • Thorough knowledge of and ability to access community resources
  • Experience in individual and group education sessions
  • Demonstrated experience in program development and coordination
  • Strong interpersonal and communication skills
  • Ability to work both independently and collaboratively as part of an inter-professional team
  • Demonstrated ability and experience in developing positive working relationships including staff, leadership, and other members of the healthcare team
  • Effective prioritization, organization, and conflict resolution skills
  • Effective decision-making and problem-solving abilities
  • Commitment to ongoing professional development
  • Computer literacy, including word processing and database management
  • Experience with Electronic Medical Record systems; EPIC experience considered an asset
  • Dedication to promoting evidence-based, patient-centered care focused on wellness
  • Understanding of consent and capacity legislation
  • Adherence to safety policies and procedures to maintain a safe and inclusive environment for all
  • Empathy in interactions with both patients and staff
