Your Opportunity:

The Navigation Senior Change Management Consultant (CMC) will work closely with the Lead of the Cancer Patient Navigation program and Supportive Care Services and Patient Experience leadership team broadly. This position will also collaborate closely with other CCA operational managers to deliver on the recommendations outlined in the Provincial CCA Navigation and Intake Evaluation Report and recommendations related to early palliative navigation outlined in the CCA Palliative Strategy. The Navigation/Intake reports outlines the need to establish four streams of specialty navigation which include Rural, Adolescent and Young Adult, Indigenous and Early Palliative. One of the early priorities highlighted in the CCA Palliative strategy is the introduction of palliative navigation services within CCA. The Navigation Change Management Consultant (CMC), provides change management, process improvement and strategic leadership for activities associated with the development, introduction and integration of the four streams of Specialty Cancer Patient Navigation roles across CCA facilities. This position will also support the alignment and integration of new Cancer Patient Navigator workflows and documentation practices in Connect Care. This position will work closely with the site based Clinical Educators, operational management, physician groups, Triage Coordinators, palliative care teams, Cancer Information System team, BEACON IT and existing Cancer Patient Navigator program within CCA to support site implementation and integration of the specialty navigators roles and resulting practice change.

Required Qualifications:

Undergraduate degree in Health Sciences. Experience in Change management, quality and process improvement methodology such as PROSCI, Lean, Six Sigma, AIW. Minimum 5 years clinical experience, preferably in the Ambulatory Cancer Care environment.

Additional Required Qualifications:

Master's degree in health sciences.

Preferred Qualifications:

Proficiency in Visio, Excel, Power Point. Excellent interpersonal, coaching skills, consultation and negotiating skills. Proven effective communication and presentation skills, analytical skills and problem-solving skills. Requires strategic and organizational leadership skills. Experience in cancer patient navigation, palliative care or care coordination.
