Job description

The Senior Clinical Manager supports the overall operations of the Bundled Care Programs in collaboration with the Area Director. Build and strengthen relationships between hospital partners & Integrated Care Solutions. Focus on building and maintaining relationships with hospital stakeholders as well as field staff to support patient care.

PRIMARY RESPONSIBILITIES:
  1. Supervise and support Clinical Managers to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
  2. Oversee and assist Clinical Managers with the integration of patient satisfaction initiatives; ensure that all referred patient concerns and complaints are satisfactorily addressed and documented in a timely manner.
  3. Liaise with hospitals, funders or Government programs as required.
  4. Liaise with Provincial regulating bodies as required.
  5. Oversee and assist with human resources with respect to recruitment and selection, orientation and training, discipline and the probationary/annual performance of field staff.
  6. Lead and assist with the implementation of Modified Work Programs for Clinical Managers.
  7. Interpret and implement relevant policies and procedures related to the Community Bundled Care Programs.
  8. Working with internal stakeholders, ensure that Programs comply with Occupational Health and Safety Standards including having active joint health and safety committees, monthly inspections and reporting incidents as required.
  9. Ensure that Programs and staff comply with Public Health standards, Ministry of Health directives and Bayshore policies related to Infection Prevention and Control.
  10. Partnering with Clinical Managers, ensure that Programs are meeting Key Performance Indicators.
  11. Ensure that cost of bundles are within acceptable levels, responsible for ensuring that overall program costs are meeting Bayshore guidelines.
  12. Ensures that Managers maintain relevant databases with patient and staff information and meet Bayshore Data Quality Standards.
  13. Participate in business development as appropriate; assist with the development of contractual agreements for client care and services as required.
  14. Participate in strategic planning and market analysis to achieve growth goals.
  15. Collaboration with Director, Community Partnerships and/or Director, Integrated Care Solutions on key and shared accounts.
  16. Attend and participate in sales, operational or external meetings as requested.
  17. Cross train staff to cover absences, as needed.
  18. Promote Bayshore Integrated Care Solutions through participation in local healthcare or business committees as well as community events.
  19. Serve as the on-call manager as required.
  20. Serve as the Clinical Manager as required.
  21. Participates in projects and champions new initiatives, assigns resources.
  22. Implements the clinical operations for any new Community Bundled Care Programs in the province of Ontario.
  23. Participate in ongoing internal and/or external continuing education activities.
  24. Adhere to Bayshore Policies and Procedures.
  25. Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  26. Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns. Responsible for completing Accident Reports for all direct reports who injure themselves on the job, within 24 hours of the incident.
  27. Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  28. Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal.
  29. Complete other tasks as requested.
  30. Exemplifies characteristics of Bayshore Health Care: compassion, excellence and reliability.
MINIMUM REQUIREMENTS:
  1. Current license in good standing as a RPN, MSW, PT, OT in province of Ontario.
  2. Graduate of an accredited and approved program.
  3. 10 – 15 years of progressive experience in health care, preferably home health care & hospital.
  4. Completed Continuing Education Requirements (CEUs) as required by regulation.
  5. Demonstrated record of building and maintaining referral relationships.
  6. Demonstrated knowledge of the Canadian health care system including; OHIP, insurance & private pay.
  7. Knowledge of medical terminology, patient care best practices and infection prevention and control.
  8. Ability to provide excellent customer service.
  9. Demonstrated ability to work well in a team environment.
  10. Working knowledge of business office technologies: PC/laptop w Microsoft Office applications, smartphone, website/internet, email.
