About the Role

We are hiring a Senior Backend Developer to take ownership of our backend architecture, powering secure consultations, smart prescriptions, health tracking, and real-time communications between patients and doctors.

This is a fully remote, freelance/contract role (with potential for long-term engagement).

Key Responsibilities

• Design, implement, and maintain RESTful APIs using NestJS and PostgreSQL
• Appointment booking & reminders
• Encrypted chat & video/audio sessions (via Agora)
• Wallet & billing system (Stripe)
• Smart prescription tracking and health metrics
• Ensure data security, compliance with PIPEDA/GDPR, and privacy best practices
• Collaborate with our frontend, AI, and mobile teams (React + Supabase)
• Contribute to API documentation (Swagger/OpenAPI)
• Implement performance optimizations, CI/CD pipelines, and testing strategies

Must-Have Qualifications

• 6+ years of experience in backend development
• Strong command of NestJS, Node.js, and PostgreSQL
• Experience with secure authentication (JWT, refresh tokens, RBAC)
• API-first development approach with Swagger/OpenAPI
• Solid knowledge of encryption, access control, and secure data architecture
• Git-based workflow and experience with CI/CD tools
• Fluent in English (written and spoken)

Nice-to-Have

• Previous experience in healthcare or medical platforms
• Familiarity with Supabase, Stripe, and Agora SDK
• Understanding of Canadian data residency requirements (AWS Canada preferred)
