We are hiring a Part-Time Registered Nurse (RN) or Registered Practical Nurse (RPN) to work in Hamilton.

Responsibilities:
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
  • When designated as the primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and support functions.
  • Support clients through care transitions by collaborating and connecting them with community support services in their local area.

Requirements:
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification is an asset.
  • Enthusiasm for community care and a passion for evidence-informed practice and excellence in care.
  • A current CPR certificate.
  • Valid driver’s license and vehicle (travel required).
  • Updated immunization records, clear criminal background check, and Vulnerable Sector Check upon hire.
  • Willingness to attain required Saint Elizabeth certifications.
  • Availability to work days (Monday to Friday, 7 am to 3 pm); no weekends.
  • This position is physically demanding and requires hand, arm, and shoulder dexterity, sensory ability, lifting up to 55 pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement.
