Job Brief

The Safer Communities Case Manager is responsible for ensuring a safe and supportive environment for youth to engage in activities that reduce the impact of criminal activity/involvement as outlined in the Building Safer Communities Fund.

Our Community Social Development Department is seeking a results-oriented team members who demonstrates professionalism and commitment to providing exemplary public service. The Safer Communities Case Manager is responsible for ensuring a safe and supportive environment for youth to engage in activities that reduce the impact of criminal activity/involvement as outlined in the Building Safer Communities Fund (BSCF).

Working in partnership with Lethbridge Police Services and under the direction of the Community Social Development Operations Manager, the Safer Communities Case Manager is responsible for ensuring a safe and supportive environment for youth to engage in activities that reduce the impact of criminal activity/involvement as outlined in the Building Safer Communities Fund (BSCF). The objective of the BSCF is to support Municipalities in their efforts to address gun and gang prevalence by providing a determined funding allocation to put in place community led projects to combat gun and gang violence and address knowledge gaps concerning the impacts of interventions in gun and gang violence.

The Case Manager will oversee all expectations of service planning including intake, admission, case plan development, evaluation, assessment of short- and long-term needs, referrals to appropriate resources and exit planning. They will work in collaboration with Lethbridge Police Services and the Community Social Development team to ensure these outcomes are achieved.

WAGE: $40.24 per hour (rate under review)

HOURS OF WORK: 7.5 hours per day, Monday to Friday, 37.5 Hours weekly

STATUS: Term position, running until March 31, 2026; subject to continued external funding

Duties And Responsibilities

  • Case management of high-risk youth, as identified by Lethbridge Police Services
  • Complete appropriate assessments according to case plans and goals
  • Case Manager’s will use a strength based and person-centered approach to develop individualized service plans that address youth’s needs including health and emotional well-being, parental/guardian support, education, safety, recreational and community supports, legal and custody, and financial needs
  • Creative case planning when developing life skills, meaningful daily activity, and connection to community
  • Provide support to parental/guardians as needed to ensure that service plan goals are met
  • Review, evaluate and document all changes in the youth and/or development
  • Provide appropriate referrals to internal and external programs for long-term support
  • Ensure all documentation and client progress information is accurate and up to date as well as weekly maintenance of caseload files
  • Actively work with client’s unique network of support by developing professional relationships with appropriate cultural or spiritual connections, family doctor, pharmacy, school districts, community resource centers and more to meet service plan goals
  • Advocate on behalf of the youth in order to meet service plan goals
  • Initiate and support Case Conferences with client support system as needed
  • Required education and experience:
  • A minimum bachelor’s degree in relevant disciplines is required with a focus on youth, health services, addiction and mental health, crime prevention, case management, etc
  • Proven experience with a minimum of 3 years’ experience in results-oriented, high paced culture focused on youth and/or case management services.
  • The ideal candidate will possess a proven track record in strategic social and/or health delivery case management, which could include clinical expertise relating to psychosocial support, addiction and mental health, or other social work.
  • An understanding of youth care practice, crisis management and trauma-informed practice is considered an asset.
  • Must have a valid Alberta driver’s license and a vehicle with appropriate auto insurance to provide transportation for program participants as needed is required for this position.
  • Experience working with youth in the social services sector
  • Intensive case management experience
  • Strong communication skills; both written and verbal
  • Strong reporting skills
  • In addition, the successful candidate will be required to complete and pass: Criminal Record and Vulnerable Sector Check, Intervention Record Check, and an extended background check (conducted by the Lethbridge Police Service).
