Qualifications

• Registered Practical Nursing Diploma required.
• Active, unrestricted registration with the College of Nurses of Ontario required.
• Experience with psychiatry or mental health settings is preferred.

Responsibilities

• Provides individualized service coordination for an assigned group of clients in collaboration with the treatment team.
• Participates in client-centered comprehensive psychiatric assessments and treatment planning.
• Administers medications and contributes to medication policy and procedure development.
• Provides on-call crisis intervention and supports night and weekend staff as required.
• Collaborates with community agencies and families to maintain coordination in the treatment process.
