Alberta Health Services seeks a Transition Coordinator RN to support patients and families as they navigate care decisions across the system. You will draw on information from multiple sources to guide transitions safely and confidently to the right place of care.

The role blends clinical insight with care coordination in a fast-paced, collaborative environment. Relocation expense reimbursement may be available; posting shows part-time temporary status with a 0.84 FTE and shifts including

Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with CRNA.
• Current BCLS-HCP certification.

Responsabilités

• Provide a wide variety of nursing services to patients, families, communities and populations.
• Utilize nursing processes, through critical thinking, problem solving and decision making.
• Lead an integrated and holistic approach to patient care, health promotion and maintenance.
• Provide safe, quality patient and family centered care.
