Job summary

Jobtailor seeks a Registered Practical Nurse to provide community-based care in Ontario. You will participate in scheduled visits, deliver ethical nursing care, and assess patients using appropriate tools within your scope.

This role involves executing and monitoring care plans, engaging patients with collaboration, maintaining records, and supporting diverse age groups at home. Travel to residences within a designated area is required, with after-hours on-call coverage.

Responsibilities

Requirements

• Experience in delivering community care is an asset

• Willingness to travel to and from patient residences within a designated area
• Experience working with the vulnerable population
• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
