CarePartners In Your Community is seeking Registered Nurses (RN) or Registered Practical Nurses (RPN) to deliver high-quality home-based nursing care across Ontario. You will develop and maintain care plans, work with patients both in-person and virtually, and provide services in diverse settings including in-home, clinic, retirement homes, or schools.

Candidates must hold an RN or RPN credential with CNO registration in good standing, and possess strong interpersonal and English communication

Qualifications

• RN or RPN degree or diploma required.
• Current CNO registration in good standing required.
• Excellent English communication skills required.
• CPR certification required.
• Immunization records including 2-step TB test required.
• Current vulnerable sector check required.

Responsabilités

• Develop and maintain an effective Care Plan for Community Health Services.
• Provide nursing services to patients in-person and virtually.
• Deliver services in-home, clinic, retirement home or school settings.
• Communicate with patients, families and care team.
• Perform other clinical duties as required.
