Qualifications

Job description

Pay Range

RPN: $22.44 - $28.99 / RN: $28.69 - $36.71

Position Overview

The Adult program supports clients aged 18 and above requiring complex care and specialized nursing skills at home. Referrals are managed through HCCSS, covering Durham, Toronto Central, and Central regions in the Greater Toronto Area. Opportunities include home visits and clinic work. A valid driver’s license and vehicle are required for visiting roles.

Responsibilities
  • Provide nursing services such as wound care, IV therapy, injections, PICC line removal, chemo disconnections, and more.
  • Develop specialized skills including G-tube feeding, TPN, pain and symptom management.
  • Support mental health and conduct client check-ins.

Key Responsibilities
  • Assess, monitor, and develop care plans, evaluating interventions through ongoing reassessment.
  • Ensure quality care by adhering to practice standards and acting on factors affecting care delivery.
  • Demonstrate professionalism, leadership, and accountability in interactions with clients and families.
  • Support client safety and continuous quality improvement practices.
  • Maintain proper documentation as per College of Nurses of Ontario standards.
Qualifications
  • Registered with the College of Nurses of Ontario (CNO) in good standing.
  • Membership in RNAO or WeRPN preferred.
  • RN or RPN diploma/degree from a recognized institution.
  • Valid First Aid or CPR certification.
  • Experience in acute care or medical-surgical nursing is an asset.
  • Knowledge of community nursing is an asset.
  • Effective communication and documentation skills.
  • Reliable attendance, punctuality, and availability.
  • Commitment to professional growth and critical thinking.
  • A valid Ontario G driver’s license and reliable vehicle are assets.
