Job summary

UNC Health is seeking a Transfer Center Coordinator – RN to coordinate inter-hospital transfers, assess patient needs, plan and evaluate care, and promote efficient flow. You will work with care teams to optimize throughput and ensure high-quality, cost-effective transfer management.

This onsite role in Chapel Hill requires a BSN, NC RN licensure, and at least three years of inpatient experience. The position is full time, 36 hours per week, with a rotating schedule and hourly compensation

Qualifications

• BSN required and NC RN licensure.
• 3 years inpatient care experience required.
• Strong organizational and communication skills.

Responsibilities

• Capacity planning and daily capacity management.
• Triaging transfer requests and developing transfer plans.
• Coordinate with referring/receiving teams and care units.
• Assess clinical appropriateness per guidelines and escalate as needed.
• Document activities accurately in supported systems.
