Overview

At Houston Methodist, the RN Clinical Communications Coordinator is a registered nurse responsible for facilitating and coordinating patient care during all phases of transplantation. This position provides professional nursing care to a diverse patient population and interacts with all members of the interprofessional transplant team to ensure coordination of care. Responsibilities include identifying transplant candidates upon referral, coordinating evaluations, assessing and triaging calls, facilitating clinic activity, and documenting all patient care issues.

Responsibilities
PEOPLE ESSENTIAL FUNCTIONS
  • Serve as the department's voice via frontline telephonic communication, modeling interprofessional communication and establishing relationships with patients and families. Identify and respond to clinical, psychosocial, and economic needs during all transplant phases.
  • Communicate with candidates, physicians, healthcare providers, payors, and team members regarding patient status. Provide telephonic education on transplant and organ donation topics.
  • Promote teamwork, anticipate team needs, offer assistance proactively, and contribute to employee satisfaction.
SERVICE ESSENTIAL FUNCTIONS
  • Follow patient- and family-centered care standards. Coordinate evaluation processes for transplant candidates and contribute to patient satisfaction goals.
  • Advocate for patients and families, addressing clinical and ethical concerns, and providing education on disease, candidacy, donation options, regulations, and process responsibilities.
  • Ensure timely communication of referral and evaluation status to patients and physicians.
  • Review patient data, communicate results, coordinate diagnostics and hospitalizations, and monitor candidate re-evaluation needs.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
  • Identify suitable candidates, evaluate contraindications, and communicate findings with the transplant team.
  • Update candidates on listing status and monitor patient status per standards. Document all interactions thoroughly and ensure compliance with regulatory requirements.
FINANCE ESSENTIAL FUNCTIONS
  • Validate insurance coverage, support financial approvals, and manage documentation to meet financial and departmental targets.
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
  • Participate in quality improvement initiatives, seek educational opportunities, and apply evidence-based practices. Maintain ongoing personal development plans.

Qualifications
EDUCATION
  • Bachelor's degree or higher in Nursing from an accredited institution.
WORK EXPERIENCE
  • Minimum of one year of nursing healthcare experience.
License/Certification
REQUIRED LICENSES AND CERTIFICATIONS
  • RN license for Texas or a compact state, obtained within 60 days.
  • OR RN-Temp license within 60 days.
  • BLS certification (AHA).
