Qualifications

• 2 year experience as a Registered Nurse or RN Case Manager.
• Bachelor of Science in Nursing degree.
• Current certificate of competence as a Registered Nurse with the College of Nurses of Ontario.
• Ability to work shifts including days, evenings, nights, weekends and statutory holidays (shift premiums available).
• Current certification in Basic Cardiac Life Support (BCLS).

Responsibilities

• Monitor hospitalized patients and/or outpatient care; update notes in the case management system.
• Secure RN assessments of medical information.
• Provide approvals for defined procedures and follow-ups; liaise with medical team as required.
• Liaise with patient/family regarding benefit information and arrangements.
• Review client policy to determine the appropriate course of action.
• Make billing arrangements and obtain updated bill amounts.
• Coordinate repatriation of clients with Transportation Services.
