This full-time, day shift Care Manager and Discharge Planner role is based at the only Level I Trauma hospital in Albuquerque, NM.

Coordinate all systems/services for an organized, multidisciplinary, patient-centered care approach, ensuring quality, cost-effective care for the patient population. Manage treatment courses, coordinate with physicians, nurses, and staff to achieve quality outcomes within set time frames and resource utilization. Conduct assessments, initiate disease management protocols, manage outcomes, ensure continuity of care through discharge planning, and analyze variances. Serve as a contact for patients, families, healthcare team, community resources, and staff. Ensure policy adherence. Patient care may include neonates, pediatrics, adolescents, adults, and geriatrics.

Responsibilities

  • Patient-Centered Medical Home: Promote core expectations.
  • Identification: Identify patients needing case management via established procedures.
  • Data Collection & Assessment: Gather and analyze clinical, psychosocial, and treatment data.
  • Needs & Resources: Assess patient/family needs, provide options, and coordinate resources.
  • Orders & Referrals: Obtain physician orders, initiate referrals.
  • Documentation: Maintain tracking systems and reports.
  • Collaboration: Build relationships with departments and community agencies.
  • Goals & Care Planning: Set patient-centered goals and develop care plans.
  • Discharge Planning & Variance Management: Conduct planning and intervene when variances occur.
  • Resource Coordination: Use resources efficiently to ensure quality outcomes.
  • Monitoring & Evaluation: Monitor responses to interventions.
  • Communication & Education: Use effective communication strategies and educate patients/families.
  • Continuity & Meetings: Ensure care continuity and participate in team meetings.
  • Quality & Development: Participate in quality improvement and professional growth activities.
  • Patient Safety & Medication: Follow safety policies, administer medications as appropriate.

Qualifications

Education: Graduate of accredited nursing program; Bachelor's degree preferred.

Experience: At least 1 year of relevant experience; bilingual skills a plus.

Credentials: RN license in NM or reciprocal; CPR certification required within 30 days.

Physical & Working Conditions

Light work exerting up to 20 pounds occasionally; may involve walking, standing, or sitting; may involve minor hazards like dust or noise.
