Summary of Duties and Responsibilities :

The Annual Wellness Visit Nurse collaborates with primary care providers and the healthcare team to identify Medicare patients suitable for annual wellness visits and executes guided processes to complete these visits. They may also assist with other clinical tasks, assess healthcare needs for follow-up, and connect patients with community resources. The role requires expertise in nursing processes, critical thinking, and accountability for patient care.

Responsibilities :
  1. Identify patients due for Medicare annual wellness visits.
  2. Coordinate with clinical teams to efficiently conduct wellness visits.
  3. Communicate effectively with patients, caregivers, and staff to enhance the patient experience.
  4. Follow protocols and complete preventive services during visits.
  5. Advocate for patients and document appropriately.
  6. Develop knowledge of Medicare guidelines and clinical recommendations.
  7. Address and close quality gaps with patients.
  8. Assist with clinical duties such as prior authorizations and prescriptions.
  9. Assess walk-in patients for urgent needs.
  10. Ensure compliance with Medicare guidelines and perform chart audits.
  11. Follow safety and HIPAA regulations.
  12. Support emergency protocols and staff training.
  13. Contribute to accreditation efforts and healthcare quality improvement.
  14. Perform other duties as assigned.
Qualifications :
  1. Current RN license in Oklahoma.
  2. At least 3 years of hospital or outpatient experience.
  3. Current BLS-HCP certification.
  4. Leadership, decision-making, communication, and computer skills.
Preferred Qualifications :
  1. BSN preferred; ADN considered.
  2. Healthcare management experience.
  3. Bilingual in English and Spanish.
Physical Requirements :
  1. Lift at least 25 pounds.
  2. Assist patients onto and off exam tables.
