Job summary

St. Joseph's Health Centre in Toronto is seeking an Registered Nurse for the Assertive Community Treatment Team (ACTT). The role focuses on psychiatric assessments, medication management with the psychiatrist, and delivering treatment in both office and community settings for patients with severe mental illness.

The RN will participate in evening shifts and rotating weekends, collaborating with an interdisciplinary team to support safety, recovery, and functional goals in the community.

Qualifications

• Bachelor's degree in nursing preferred; psychiatry/community nursing asset.
• Current Certificate of Competence with the College of Nurses of Ontario.
• 1 year of recent community mental health experience preferred.
• 3 years of clinical work with severe mental health issues preferred.
• Ability to collaborate with clients, families and supports.

Responsibilities

• Coordinate care for clients with severe mental illness and plan care actions.
• Provide case management, crisis intervention, and medication management.
• Communicate effectively with client, family, ACTT workers and providers.
• Monitor physical health and side effects; arrange referrals as needed.
• Liaise with pharmacies, labs, and community services; support discharge planning.
