Job summary

Sunnybrook Health Sciences Centre is seeking a qualified professional to join the Assertive Community Treatment Team, providing client-centred treatment, rehabilitation, support and case management to individuals with severe and persistent mental illness within the community.

The role involves working in a multidisciplinary team within an ACT framework, coordinating care, and traveling within the catchment area to reach clients in need.

Qualifications

• MSW, BSc OT, MSc OT, or COAT approved Equivalent required.
• Member in good standing of the Ontario College of SW and SSW, or College of Occupational Therapist of Ontario required.
• Recent community experience working with patients with severe and persistent Mental Illness.
• Knowledge and comfort with functioning within the ACT model of Care.
• Basic knowledge of psychotropic medications.
• Ability to work independently and interdependently within a multidisciplinary team.
• Excellent oral and written communication skills.
• Demonstrated effective interpersonal skills.
• Demonstrated problem-solving skills.
• Commitment to client-centered care.
• Knowledge of the health care system and community resources.
• Valid Ontario Driver’s License required.
• Ability to travel extensively within the catchment area.

Responsibilities

• Provide client-centred treatment, rehabilitation, support, and case management in the community for clients with severe and persistent mental illness.
• Collaborate within a multidisciplinary team to plan and implement care.
• Utilize the ACT model of care to coordinate services and resources.
• Maintain accurate documentation and communication with clients, families, and providers.
• Travel within the catchment area to reach clients where they live and receive services.
