Job description

Schedule: 8:00am-4:30pm; every other weekend

Why You Should Be Our Next Resource Specialist:

You will collaborate with Nurse Care Coordinators, Social Workers, and other care team staff to ensure patients receive the necessary resources and services for a successful return to a community setting, including home with services (e.g., Visiting Nurse Association), without services, long-term care facilities, community hospitals, or outpatient clinics.

The Resource Specialist is responsible for developing educational and referral information regarding extended care facilities, placement processes, homecare, and community resources. This includes understanding levels of care, long-term care assessment forms, and Medicare/Medicaid regulations related to placement.

This role involves advocating for patients and their families, supporting hospital goals for resource management, customer satisfaction, and quality care.

Minimum Qualifications:
  1. High School Diploma or Equivalent required
  2. Associate's Degree or Bachelor's Degree preferred
  3. Healthcare experience in extended care facilities and community agencies preferred
  4. Experience in hospital discharge planning, long-term care, community health, or utilization review preferred
  5. Knowledge of medical terminology preferred
  6. Adherence to all I C.A.R.E. Standards
  7. Effective interpersonal skills to interact with staff, patients, families, and community organizations
  8. Ability to participate in an interdisciplinary team
  9. Extensive knowledge of regulations, community organizations, healthcare systems, and medical terminology
  10. Ability to manage a variable workload and adapt to changing priorities
  11. Proactive and independent work skills
  12. Basic typing and computer skills; experience with software is desirable
