An established industry player is seeking a dedicated Resource Specialist to enhance patient care through effective resource management and community collaboration. In this role, you will work closely with Nurse Care Coordinators and Social Workers to ensure patients have access to vital services for their transition to community settings. Your expertise in healthcare systems and regulations will be crucial as you advocate for patients and their families, ensuring quality care and satisfaction. Schedule: 8:00am-4:30pm; every other weekend
You will collaborate with Nurse Care Coordinators, Social Workers, and other care team staff to ensure patients receive the necessary resources and services for a successful return to a community setting, including home with services (e.g., Visiting Nurse Association), without services, long-term care facilities, community hospitals, or outpatient clinics.
The Resource Specialist is responsible for developing educational and referral information regarding extended care facilities, placement processes, homecare, and community resources. This includes understanding levels of care, long-term care assessment forms, and Medicare/Medicaid regulations related to placement.
This role involves advocating for patients and their families, supporting hospital goals for resource management, customer satisfaction, and quality care.
