The Resident & Family Experience Coordinator III is a vital role within our communities, impacting residents' lives and fostering meaningful human connections.

As a Resident & Family Experience Coordinator III, your typical day will include:

1. Leading the resident move-in, transfer, and move-out processes in accordance with the organization’s standards, including facilitating applications, assisting with financial support access, conducting tours, and introducing residents and their families to the community and its operations.
2. Providing social work counselling services, collaborating with the inter-professional team to develop care plans that address psychosocial and social needs, promoting overall wellbeing and safety.
3. Planning, developing, implementing, and evaluating individual social work services and support groups to meet residents' and families' needs, engaging external resources as necessary.
4. Supporting residents and families during advanced care planning and treatment discussions, including funeral arrangements.
5. Recruiting and supervising social work students’ placements.
6. Responding to, tracking, and addressing feedback received in the resident area to ensure timely and satisfactory resolutions.
7. Providing reporting and analysis of feedback to Leadership & the Quality Committee.

Must Haves

• Registered Social Worker per provincial legislation
• Minimum of three years’ experience in healthcare, social services, or a related sector
• Knowledge of the Long Term Care Act & Regulations (preferred)
• Customer service experience (asset)
• Hospitality and/or sales experience (asset)
• Strong oral and written communication skills
• Ability to organize, plan, and delegate responsibilities
