DEPARTMENT: Resident Care (Nursing)

JOB TITLE: Care Manager

REPORTING RELATIONSHIP: Site Leader and/or Director of Care

QUALIFICATIONS: Baccalaureate of Nursing with post basic course in Gerontology or three (3) years recent related experience or an equivalent combination of education, training, and experience. Current practicing registration with the applicable provincial registering body.

JOB SUMMARY:

The Care Manager is responsible for recruiting and leading the nursing care team in providing care and services for Residents. They liaise with Residents’ families, physicians, and Health Authority representatives, and coordinate and supervise the nursing team to ensure Resident-centered care based on individualized needs and care plans. The Care Manager demonstrates leadership and gerontology best practices in accordance with provincial regulations, health mandates, and Park Place policies, with a strong commitment to Resident safety and a culture of safety.

TYPICAL DUTIES AND RESPONSIBILITIES:
  1. Recruits, orients, leads, and supervises the nursing team to ensure effective Resident care planning and delivery.
  2. Ensures comprehensive assessments are completed and used for individualized care planning, including evaluation and revisions as needed.
  3. Provides leadership, mentorship, and performance evaluations to nursing staff; offers ongoing education and addresses performance issues.
  4. Acts as a role model demonstrating professionalism, empathy, trust, and respect.
  5. Establishes therapeutic relationships with Residents, families, staff, and others.
  6. Supports advocacy for Resident autonomy and decision-making.
  7. Maintains appropriate staffing levels and monitors team collaboration for quality care.
  8. Leads quality improvement initiatives and ensures documentation standards are met.
  9. Plans and organizes team activities, responds to changing needs, and ensures safety and infection control protocols are followed.
  10. Participates in educational programs to maintain clinical competence.
  11. Performs other duties as assigned.
HEALTH AND SAFETY RESPONSIBILITIES:
  • Complies with Occupational Health and Safety policies and educates staff annually on workplace violence and harassment.
  • Promotes and monitors safe work procedures, reports incidents, and corrects unsafe conditions.
  • Leads by example in safety practices.
SKILLS AND ABILITIES:
  • Fluent in English, both verbal and written.
  • Demonstrates leadership, emotional maturity, and motivation skills.
  • Possesses strong problem-solving, clinical judgment, teamwork, professionalism, change management, and communication skills.
  • Proficient with computer systems and nursing equipment.
PHYSICAL DEMANDS:

Requires lifting up to 50 lbs, walking, bending, and standing for extended periods, with frequent bending or crouching.
