Job summary

The Salvation Army is seeking a Supported Housing Program Manager to lead care delivery across the program, oversee comprehensive care planning, and coordinate interdisciplinary teams to ensure safe, person-centred resident care. You will guide LPNs and HCAs, support quality improvement, and work with residents and families to advocate for choice and dignity in a compassionate, mission-driven environment.

Responsibilities

• Direct and monitor care services to achieve safe, resident-centered care.
• Develop plans and strategies for care services aligned with organizational directions.
• Support complex case navigation together with the interdisciplinary team.
• Evaluate care services using defined objectives and measures for quality improvement.
• Ensure admissions, transfers, and discharges follow policy and contractual obligations.
• Provide advocacy and communication to residents and families.
• Develop and evaluate policies and procedures; ensure compliance with standards.
