Job Summary & Requirements
Reporting to the Manager, Clinical Pharmacy Services, the Renal Pharmacist is an integral member of the Pharmacy Team and the Renal Program. The Renal Pharmacist supports out-patients in the renal program, including hemodialysis, peritoneal dialysis, and patients in the Kidney Clinic. They are responsible for completing patient medication assessments, ensuring optimal medication therapy, and minimizing potential adverse events. They also provide accurate medication information to the interdisciplinary team, promoting safe and appropriate medication use to minimize risk and optimize healthcare resources.
Responsibilities:
- Review and update patient medication lists, review bloodwork, and make recommendations to nephrologists.
- Complete patient medication assessments.
- Ensure optimal medication therapy and minimize potential adverse events.
- Review lab information to ensure appropriate drug therapy.
- Participate in patient rounds in the clinical area of practice.
- Participate in drug distribution, including accurate order entry of physician orders.
- Provide healthcare professionals with relevant drug information.
- Review and provide recommendations on document submissions related to clinical coverage.
- Work collaboratively with the interdisciplinary team.
- Conduct medication safety audits aligning with Accreditation Canada standards.
- Participate in education and mentoring of new staff, including precepting pharmacy students and residents.
- Contribute to the education of nursing staff.
- Perform other duties as assigned.

Education and Licensing:
- Registered Pharmacist in good standing with the Ontario College of Pharmacists (Part A).
- Bachelor of Science or PharmD degree in Pharmacy.
Experience:
- Post-graduate Pharmacy Residency Program (ACPR) is an asset.
- Minimum two years of hospital pharmacy experience preferred.
Competencies:
- Knowledge of evidence-based pharmacotherapeutics related to kidney disease.
- Commitment to ongoing professional development as per Regulated Health Professions Act.
- Understanding of drug dosing and pharmacokinetics in chronic kidney disease.
- Knowledge of medication dosages, delivery techniques, and routes of administration.
- Interpretation of laboratory and clinical parameters for patient assessment in kidney disease.
- Proficiency with electronic and print medical information resources.
- Understanding of third-party reimbursement and drug coverage for end-stage renal disease.
- Knowledge of health promotion, disease prevention, and complementary medicines.
- Familiarity with aseptic techniques and pharmaceutical preparation.
- Awareness of multiculturalism and diversity.
- Proficiency with Microsoft applications.
- Ability to interpret research methodology and critically appraise studies.
- Excellent time management and organizational skills.
