Job summary

Horizon Health Network in New Brunswick is seeking a Regional Director of Population Health to join our team. You will lead population health initiatives, drive system transformation, and address health disparities across Horizon’s network.

The ideal candidate holds a master’s degree in health or social science, with 7–10 years of progressive leadership experience, and strong organizational, HR and financial management skills.

Qualifications

• Master’s degree in health or social science.
• 7–10 years progressive leadership experience.
• Strong organizational and time management skills.
• Experience in human resource and financial management.
• Two-Eyed Seeing approach and collaboration experience (asset).

Responsibilities

• Develop and execute strategic plans for Population Health in line with Horizon’s Strategy.
• Provide strategic leadership for evidence-informed population health approaches.
• Identify and support research, best practices and projects aligned with Horizon.
• Support Two-Eyed Seeing wellbeing approaches for Indigenous clients.
• Serve as a strategic business partner to influence organizational levels.
• Collaborate with Department of Health and external stakeholders to modernize population health services.
• Lead regional evidence-based practices in community engagement with partners.
• Supervise multiple regional population health teams.
• Oversee quality, safety and budget reporting across Population Health.
