JOB OVERVIEW

The Reminiscence Coordinator is responsible for providing overall leadership and management of the reminiscence neighborhood. Responsibilities include promoting excellence in resident care in accordance with each resident’s Individualized Service Plan (ISP)/Individualized Care Plan (ICP), programming, providing outreach and support to families, and being resourceful with the Family Resource Library and Alzheimer’s disease information. The role also involves championing the care manager concept, assisting in recruiting, hiring, training team members, and overseeing staffing, scheduling, and performance management.

RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:
  • Support Sunrise’s Mission, Principles of Service, and Core Values, emphasizing the value of human life.
  • Lead the operation of the reminiscence neighborhood(s) in compliance with laws, standards, and regulations.
  • Champion the Resident Centered model and reminiscence programming.
  • Collaborate with the Activities and Volunteer Coordinator to ensure diverse, individualized activities are available and engaging for residents.
  • Design and facilitate dementia-appropriate activities focusing on creating pleasant days.
  • Partner with the Resident Care Director to address residents’ clinical needs.
  • Review resident preferences and needs before move-in and facilitate Family Meetings.
  • Develop and oversee the ISP/ICP and Daily Assignment Sheets to ensure quality service delivery.
  • Manage assessments to determine care levels and staffing needs.
  • Plan family events and services.
  • Monitor resident condition changes and communicate appropriately with families and physicians.
  • Document pertinent information in Daily Logs and establish relationships with Alzheimer’s organizations.
  • Coordinate educational support and facilitate family support groups.
  • Ensure compliance with clinical indicators and safety standards.
Quality Assurance and Regulatory Compliance:
  • Ensure community compliance with all applicable laws and standards.
  • Develop corrective action plans for deficiencies.
  • Stay informed of current laws and regulations and ensure adherence.
  • Promote safety procedures and OSHA compliance.
Financial Management:
  • Assist in presenting community services and in budget preparation.
  • Manage department budget, review financial statements, and control expenses.
  • Coordinate staffing to optimize resources and cross-training.
Training, Leadership, and Team Member Development:
  • Manage recruitment, supervision, training, coaching, and discipline of staff.
  • Ensure completion of all required documentation and training.
  • Participate in Sunrise University and present training modules.
  • Achieve team engagement goals and conduct performance reviews.
  • Attend meetings and stay updated on professional developments.
Qualifications:

Candidates should have a college degree (preferred), supervisory experience (preferred), and the ability to handle multiple priorities. Effective communication, organizational, and computer skills are essential. Flexibility to work weekends, evenings, and various hours is required.
