Job Overview

The Reminiscence Coordinator is responsible for providing overall leadership and management of the reminiscence neighborhood. Responsibilities include promoting excellence in resident care according to each resident’s Individualized Service Plan (ISP)/Care Plan (ICP), programming, supporting families, and being resourceful with Alzheimer’s information. The role involves championing the care manager concept, recruiting, training staff, and managing staffing, scheduling, and team engagement.

Resident Care And Programs

Supporting our Mission, Principles of Service, and Core Values, with responsibilities including:

  • Leading the operation of the reminiscence neighborhood(s) in compliance with laws and Sunrise standards.
  • Promoting quality delivery of the Resident Centered model.
  • Acting as the community champion for reminiscence programming and activities.
  • Partnering with the Activities and Volunteer Coordinator to ensure appropriate activities are available and residents are engaged.
  • Designing and facilitating dementia-appropriate activities focusing on creating pleasant days.
  • Collaborating with the Resident Care Director to address residents’ clinical needs.
  • Reviewing resident preferences and needs prior to move-in and scheduling Family Meetings.
  • Facilitating the development of ISP/ICP and Daily Assignment Sheets for quality service, safety, and engagement.
  • Managing assessments to determine care levels and staffing needs.
  • Planning family events and services.
  • Monitoring resident condition changes and communicating with families and physicians.
  • Documenting pertinent resident information in Daily Logs.
  • Establishing relationships with Alzheimer’s organizations and community groups for educational support and family meetings.
  • Ensuring clinical indicators are met and below benchmarks for restraint use, weight management, and behavior management.
Quality Assurance And Regulatory Compliance
  • Ensuring community compliance with all relevant laws and standards.
  • Developing plans to correct deficiencies and ensure ongoing compliance.
  • Staying informed of evolving laws and regulations.
  • Promoting safety procedures including PPE and OSHA compliance.
Financial Management
  • Supporting community product and service presentation.
  • Assisting in community budgeting and managing departmental expenses.
  • Reviewing financial statements and implementing improvement plans.
  • Managing expenses and staffing to optimize community resources.
Training, Leadership, and Development
  • Managing staff recruitment, training, supervision, and performance.
  • Ensuring all team members complete required paperwork and training.
  • Participating in Sunrise training programs and delivering team education.
  • Conducting performance reviews and fostering team engagement.
  • Attending meetings and staying updated on professional developments.
Qualifications
  • College degree preferred; supervisory experience in dementia care is advantageous.
  • Ability to handle multiple priorities and delegate effectively.
  • Excellent communication, organizational, and judgment skills.
  • Proficiency in computer applications including Microsoft Office and Sunrise systems.
  • Availability to work weekends, evenings, and flexible hours.
