Job description

POSITION TITLE: RELIEF ACCESS COORDINATOR

LOCATION: 54 Wolseley Street, Toronto, ON - On-site

APPLICATION CLOSING DATE: February 21, 2025

POSITION TYPE: CASUAL - (0 - 80 hours per month)

ORGANIZATION:

Stella’s Place provides mental health service for young adults (16-29 years). We utilize innovative clinical practices and processes that put young adults in the ‘driver’s seat’ of their own recovery plan. In this role, you will work as a member of our multidisciplinary team to facilitate and support participants with their recovery process. Stella’s Place focuses on integrating peer support with evidence-based treatment using modalities such as Dialectical Behaviour Therapy (DBT), counseling, and recovery supports.

POSITION:

This position is an exciting opportunity to support Young Adults engaged with Stella’s Place programming and is central to the success of Stella’s Place. You will be the first point of contact for young adults, service providers, families, donors, and partners who contact Stella’s Place through email, phone, or in-person. You will work independently as well as with a multi-disciplinary team including but not limited to Peer Supporters, Development, Leadership and Clinicians.

Skilled at assessing needs, your work will include a significant administrative component, ensuring appropriate documentation and maintenance of information and record-keeping systems.

PURPOSE:

The purpose of this position is to provide information, access, and intake services to young adults and their supporters. You will work with young adults to identify and assess their needs, ensure that they are connected with internal and external resources and support young adults accessing our open spaces.

RESPONSIBILITIES:

Access Coordinator responsibilities include but are not limited to:

  1. Act as the initial point of contact for young adults and their families as well as other mental health professionals helping young adults to access care
  2. Answer phone, email, and in-person inquiries from young adults and stakeholders
  3. Share information about Stella’s Place with young adults, service providers, stakeholders or family members who inquire about our services
  4. Assess the needs of young adults and families who are contacting Stella’s Place in order to triage them effectively
  5. Respond effectively to young adults and/or families who are in distress or experiencing crisis, in a self-regulated manner
  6. Provide information and where appropriate make referrals to support young adults and families in accessing internal and external resources
  7. Conduct individual intake appointments and some group intake follow-up appointments
  8. Accept and record new participant information in our electronic health record (EHR)
  9. Share management of bookings for the psychiatrist, family physician, and drop-in counseling sessions
  10. Ensure coverage and monitoring of the young adults accessing our drop-in Café program
  11. Utilize a young adult-directed recovery approach along with culturally-responsive, trauma-informed and anti-oppressive approaches
  12. Collaborate with Stella’s Place team to meet the needs of participants
  13. Provide administrative support to the weekly group drop-in counseling program
  14. Maintain required documentation in a clear, professional, and timely manner
  15. Contribute to the development of programming with program staff, young adults, and support the endeavours of the Access and Clinical Team
  16. Participate in programming that balances both traditional, evidence-informed clinical practice with young adult-driven initiatives
  17. Complete needs assessments, safety assessments and safety planning as needed
  18. Attend Team Meetings and Staff Meetings
  19. Participate in educational activities and initiatives to enhance the skill, experience, and knowledge of Stella’s Place staff
  20. Refer to external resources as required
  21. Report to the Manager of Clinical and Recovery Services
  22. Other duties as assigned

QUALIFICATIONS REQUIRED:

  1. Completion of a degree/diploma in Social Work/Social Service Worker/Human Services or equivalent
  2. Demonstrated experience working in a mental health, health, or community setting in an administrative role (minimum 1-year experience)
  3. Experience providing services to young adults 16 – 29 years of age (minimum 1-year experience)
  4. Demonstrated ability to respond effectively to distress while remaining self-regulated
  5. Demonstrated creative thinking in addressing service delivery issues
  6. Demonstrated ability to connect young adults to community resources
  7. Demonstrated experience with multitasking and time management
  8. Demonstrated problem-solving ability, sound judgement, and assessment abilities
  9. Demonstrated experience working with diverse populations
  10. Demonstrated experience working collaboratively as a member of a multidisciplinary team
  11. Ability to work independently with little supervision
  12. Familiarity with Peer Support and clinical treatment approaches such as DBT, CBT or Brief Solution Focused Therapy is an asset
  13. Experience with using Client Information Management Systems and Electronic Health Record management
  14. Demonstrated Knowledge of Privacy and Confidentiality legislation and practices

  7. Front desk/receptionist experience and/or medical secretary experience

BACKGROUND:

A Vulnerable Sector Screen is required to be submitted to Stella’s Place before start date without exception.

REPORTING:

Communication, coaching, mentoring, personal and professional development, and evaluation will take place with one of the program team Managers on a regular basis. The occasional weekend may be required.

SALARY:

Hourly Rate - $25/hr plus 4% vacation pay
