The opportunity:

We are currently looking to fill a Full-time, Permanent Remote Rehabilitation Services Coordinator role.

Responsibilities:

  • This role requires excellent writing skills, professional communication, strong organizational skills, and high attention to detail. The candidate should work well independently and collaborate effectively with the team.
  • Works with the supervisor to monitor program resources, as well as human and financial needs to meet program goals and objectives
  • Recruits, employs, trains, supervises, and evaluates support service staff within a pre-determined budget in accordance with the approved program plans
  • Coordinates the assessment, development, and implementation of Individual Rehabilitation plans or Individualized Care plans with assigned staff
  • Creates Quarterly Progress Reports, Individualized Rehabilitation Plans, and Care Plans based on client goals, functional abilities, and progress, with attention to detail and deadlines
  • Participates as a member of the community support services administration team in planning, problem resolution, mediation, quality management system/improvement, and review of service delivery performance
  • System navigation and case coordination, liaising with numerous and diverse external agencies, services, funders, and community-based resources
  • Responsible for all service-related documentation - coordinating with clients, families, funders, and substitute decision makers
  • Manages client files in accordance with organizational policy
  • Coordinates and leads meetings as required
  • Acts as a staff resource to support activities of Supervisors and Program Managers

Qualifications and experience:

  • 2 – 5 years’ experience in the Social Services/Health Sector
  • University or College degree/diploma and/or equivalent experience
  • Experience with reporting and coordinating services for individuals with disabilities
  • Community-based knowledge/experience working with individuals with brain injury, mental health, physical disabilities, or special needs
  • Demonstrated experience in team leadership
  • Working knowledge of third-party funders and corresponding legislation
  • Highly organized and excellent time management skills
  • Demonstrated ability to train employees, including organizing, prioritizing, and scheduling work assignments
  • Problem solving and conflict resolution skills
  • Knowledge of human resource practices, behavior management principles, and Health & Safety legislation
  • General understanding of computer software including Microsoft Office
  • Strong ability to assess and determine effective strategies to support independent living goals

Preferred location: Edmonton, Winnipeg, Ottawa, Halifax
