Job description

We are seeking casual staff for the position of Rehabilitation Care Worker in Vancouver and Burnaby.

Job Summary :

In consultation with the Program Manager, the Rehabilitation Care Worker supports persons with disabilities by providing life skills training and support services for clients regarding issues such as personal care, access to community leisure programs, housekeeping duties, planning and preparing meals, booking medical appointments and organizing a weekly schedule. The Rehabilitation Care Worker empowers the client(s) to perform all aspects of daily living as independently as possible, providing support and intervention where necessary.

Job Duties :

  • Observes and reports on clients’ needs, performance, and progress reporting on medical, behavioral, and other concerns as required. Provides intervention as required in accordance with established care plans.
  • In accordance with established Individual Service Plans and rehabilitation schedules, assists clients with the development of life skills such as maintaining personal hygiene, housekeeping, personal laundry, meal planning and preparation, meeting financial obligations / budgeting, making and keeping appointments, and following a daily or weekly schedule.
  • In accordance with established Individual Service Plans, assists clients with interpersonal skills by methods such as demonstrating and modeling appropriate actions, and with the development of cognitive and physical skills using compensatory strategies, such as memory and physical aides, where applicable.
  • Administers medication to clients and provides medication reminders, in accordance with established policy and Individual Service Plans.
  • Participates in and oversees various client-focused social and recreational activities.
  • Collaborates with the client(s) to identify available social, economic, recreational, and educational services and resources in the community that will meet clients’ needs. Provides clients with related information.
  • Assists clients with activities of daily living such as feeding, transfers, grooming, toileting, and exercise programs when required.
  • Accompanies and assists clients on outings such as appointments, shopping, vocational and leisure activities, and assists client in arranging transportation to and from these activities.
  • Completes and maintains related records and documentation such as Individual Service Plans, client admission forms, statistics, progress reports, daily logs, checklists and financial records.
  • Receives client feedback, and inquiries and complaints, and responds and reports concerns to management in accordance with established policy.
  • Performs the duties of a key worker when required.
  • Performs housekeeping duties such as sweeping and mopping floors, vacuuming, dusting, washing dishes, and laundry.
  • Performs outdoor residence maintenance as required.
  • Provides direction to volunteers as required.
  • Performs other related duties as assigned.

Qualifications :

  • Recent related experience working with the brain injured or other disabled population in a group home setting
  • Current certification in Emergency First Aid or equivalent, CPR level A
  • Relevant post-secondary education or equivalent combination of education, training and experience
  • Proficient in written and spoken English

Starting Rate :

  • 24.76 / hour overnight

Burnaby Availability : AM shifts between 8 am - 6 pm, PM shifts between 4 pm - 12 am, Overnight shifts between 12 am - 8 am

Vancouver Availability : AM shifts between 7 am - 5 pm, PM shifts between 3 pm - 11 pm, Overnight shifts between 11 pm - 7 am
