The Rehabilitation Assistant gathers knowledge about residents in order to respond to their individual needs through a process of formal and informal assessments. They will collaborate with nursing, physiotherapy, restorative care assistant, resident and families to ensure that consistent progress is made toward their physical and quality of life goals. The Rehabilitation Assistant manages and maintains the Providence Manor equipment inventory, coordinates with outside vendors to support resident mobility and assistive device needs to ensure optimal quality of life and comfort are achieved. The Rehabilitation Assistant prioritizes referrals, communicates through care plans and with families, residents, community partners and team members regarding policies & procedures for equipment, supportive devices, maintaining equipment and safety. A resident centered approach is at the forefront of all interactions with the goals of maintaining and supporting mobility, independence, optimal levels of functioning, safety and quality of life.
Skills And Qualifications Required
- Occupational Therapy/Physiotherapy assistant diploma
- One year LTC experience
- DementiAbility and GPA preferred
Position Responsibilities
- Assesses all residents in order to respond to their needs to support an optimal quality of life and care plans accordingly
- Maintains a wellness focus
- Supports interprofessional care team with all assistive devices (i.e. mobility aides, pressure relief, toileting supports, dietary aides, etc.)
- Manage and maintain equipment inventory, bed entrapment logs, current restraints
- Coordination of vendor services, support equipment repairs (including maintenance log), quotes and support temporary equipment usage
- Educate, communicate and support interprofessional team, resident and families on all mobility and supportive devices
- Assists residents in the use of restorative aides for ambulation, including canes, walkers or crutches
- Prioritize referrals and delegate as appropriate (i.e. physiotherapist)
