The Opportunity
Are you passionate about helping others live their best lives? As a Rehabilitation Assistant, you will work under the direction of a Rehabilitation Therapist (OT/PT) to support residents in achieving their health and mobility goals. Your role will include assisting with range-of-motion exercises, client mobilization, safe transfers and lifts using appropriate aids, gait training, leading group exercise activities, and preparing both residents and equipment for therapy sessions.
What You’ll Do
Support therapeutic interventions by administering manual exercises and guiding residents through physical activities such as non-manual exercises, ambulatory functions, and daily living activities (DAL).
Manage rehabilitation equipment by assessing, installing, maintaining, and auditing devices such as transfer poles, wheelchairs, bedrails, and alarms, and completing related documentation.
Monitor and report resident participation and response to therapy interventions.
Review communication logs and daily reports to stay informed of care updates.
Collect relevant data and help identify emerging issues or trends in resident care.
Observe and promptly report any changes in medical status, behavior, or potential for aggression to the Therapist and/or Manager.
Collaborate closely with the care team (RN, LPN, Rehabilitation Therapist) to ensure seamless communication and support for each resident’s care plan.
What You Bring
Completion of a Rehabilitation Assistant Diploma from a recognized institution or an equivalent combination of education and experience.
Previous experience in continuing care, gerontology, or dementia care is strongly preferred.
A resident-centered mindset, with the ability to build trust and use motivational techniques effectively in a long-term care environment.
Proven ability to work both independently and collaboratively in a multidisciplinary team.
Excellent communication and interpersonal skills. Experience with documentation systems like POC is an asset.
Proficiency in English, both verbal and written, is essential.
