Rehabilitation Assistant – Community Advantage Rehabilitation (CAR) – a division of Lifemark

Employment Type: Independent Contractor

Location: Port Perry, Uxbridge- Durham Region, ON

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation(CAR) is currently seeking qualified Rehabilitation Assistants to provide services to our clients within the community.

At Community Advantage Rehabilitation, we work together with our clients to advance their quality of life using best practices, innovation, and quality standards.

Primary Responsibilities:

  • Deliver prescribed treatment plan as assigned by the supervising therapist to individuals living in the community
  • Perform various treatments as prescribed and directed by the supervising therapist including, but not limited to, gait training, balance exercises, transfer training, range of motion, strengthening, application of modalities (i.e. ultrasound, TENS), ADL retraining, cognitive retraining, wheelchair skills, etc.
  • Deliver dynamic, safe, and therapeutically beneficial home exercise programs
  • Maintain regular ongoing communication with the supervising therapist regarding any changes or concerns about a client's status or treatment interventions
  • Effectively communicate information to client/caregivers
  • Complete and maintain current and accurate documentation related to client’s progress using EMR technology
  • Understand and implement infection prevention practices

Requirements:

  • Graduate of a recognized OTA/PTA program (required)
  • Experience working within a community setting is an asset but RAs with general experience are also encouraged to apply
  • Ability to work independently and as part of an inter-professional team
  • Outstanding focus on safety and ethical standards
  • Strong ability to build rapport with clients
  • Ability to prioritize, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written, and report writing skills
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation is required for this position
