Rehabilitation Assistant – Community Advantage Rehabilitation (CAR) – a division of Lifemark

Employment Type : Part Time hours

Location : Port Perry, Uxbridge- Durham Region, ON

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR), is seeking qualified Rehabilitation Assistants to provide services within the community.

Since 1997, we have been providing community rehabilitation services for adults and children in Durham Region. Our vision is to be a trusted healthcare provider, helping clients maximize their functional abilities and participate fully in life.

Primary Responsibilities :

• Deliver prescribed treatment plans assigned by supervising therapists to clients in the community
• Perform treatments such as gait training, balance exercises, transfer training, range of motion, strengthening, modalities (ultrasound, TENS), ADL and cognitive retraining, wheelchair skills, etc.
• Implement safe and effective home exercise programs
• Maintain communication with supervising therapists regarding client status and treatment
• Communicate effectively with clients and caregivers
• Accurately document client progress using EMR technology
• Follow infection prevention practices

Requirements :

• Graduate of a recognized OTA/PTA program (required)
• Community setting experience is an asset, but general experience is also encouraged
• Ability to work independently and in a team
• Strong focus on safety and ethics
• Excellent rapport-building skills
• Ability to prioritize and meet deadlines
• Excellent communication and report-writing skills
• Proficiency with computers
• Strong organizational skills
• Reliable transportation
