Job summary

Extendicare is seeking a Physiotherapy/Occupational Therapy Assistant to provide therapeutic measures based on individual resident needs in a long-term care setting. You will work under the supervision of the Director of Care and guidance of the Occupational Therapist to develop and implement therapy programs.

You will collaborate with the clinical team, lead staff in therapy-related care, and maintain comprehensive resident records while promoting a safe and supportive environment for residents

Qualifications

• Must be a graduate of an approved Physiotherapy/Occupational Therapy Assistant Certificate program from a recognized Community College.
• Current experience in a long-term care setting.
• Oral and written fluency in English.
• Proficiency in a second language would be considered an asset.
• Demonstrated knowledge of Physio/OT practices and techniques in Long Term Care.
• Willingness to pursue further education in relevant specialties.

Responsibilities

• Develops, implements and evaluates individualized therapy programs with the Occupational Therapist.
• Coordinates stretch and strut ambulation program and monthly OT calendar-related classes.
• Provides daily therapy as instructed in the absence of the OT.
• Evaluates resident status and coordinates referrals with the clinical team.
• Instructs staff on ongoing care requirements for residents related to therapy programs.
• Maintains up-to-date individual resident care plans through interdisciplinary care conferences.

Purpose

Under the supervision of the Director of Care and under the direction/guidance of the Occupational Therapist, provide therapeutic measures based on individual needs.

• Bi-annual Performance Appraisal

Key Accountabilities

• In conjunction with the Occupational Therapist, develops implements and evaluates individualized therapy programs.
• Co-ordinates current stretch and strut ambulation program and physio/exercise classes, as outlined on the monthly OT calendar.
• Provides daily therapy as instructed on the absence of Occupational Therapist.
• Provides ongoing evaluation of resident status.
• Collaborates with clinical team to initiate referrals.
• Instruct staff to ongoing care requirements for residents related to therapy programs
• Develops and maintains up-to-date individual resident care plans through interdisciplinary care conference and care planning.
• Maintains accurate and complete clinical records on all residents according to established documentation policies and procedures and ensure confidentiality of these records.
• Assists in maintaining a physical and psychosocial environment which meets the needs of the residents.
• Promotes a positive team spirit within the department and the home
• Attends in-service education in order to keep informed of current trends in nursing practice.
• Participates in the home’s learning needs assessment.
• Follows directives/memos/policies within the Home.
• Is aware of and promotes the Resident Bill of Rights and incorporates it in daily practice.
• Demonstrates a sense of social and moral responsibility and upholds a just and fair code of business conduct in accordance with the Code of Conduct of Revera.. Behaves in a manner consistent with the values of the Company.
• Performs other duties as assigned.

Health and Safety

• Recognizes safety hazards and ensures appropriate action is taken, adhering to established policies and procedures including safety rules and safe working practices.
• Is responsible for proper care of equipment used in providing care to residents.
• Participates in organized fire drills and ensures safety measures for all personnel.
• Maintains a good attendance record according to company policy.

• Reports to: Occupational Therapist/Director of Care.

Professional Specifications

• Must be a graduate of an approved Physiotherapy/Occupational Therapy Assistant Certificate program from a recognized Community College.
• Current experience in a long-term care setting.
• Oral and written fluency in English.
• Proficiency in a second language consistent with the Resident population would be considered an asset.
• Must have demonstrated ability with comprehensive knowledge of Physio/OT practices and techniques as they relate to Long Term Care Resident at various levels of cognitive functioning.
• Willingness to pursue further education in relevant specialties.
