Job description

Purpose

The Seniors Mental Health Program provides specialized community-based mental health services to older adults across the District of Algoma. The program supports individuals experiencing mental health challenges that may impact their independence, daily functioning, safety, or relationships. As a key member of this interdisciplinary team, the Social Worker will provide comprehensive psychosocial assessments, develop individualized care plans, and offer support and education to clients and caregivers. The Social Worker will also collaborate with team members and facilitate access to geriatric psychiatry consultations when needed, with the goal of promoting stability, well-being, and improved quality of life for seniors.

Duties

  • Serve as a liaison between the Mental Health and Addictions Program and long-term care facilities, hospitals, and community agencies.
  • Provide psychosocial assessments and support for older adults with mental health concerns in various settings, including home visits.
  • Collaborate with psychiatrists, nurses, primary care providers, and other team members to develop, implement, and evaluate comprehensive care plans.
  • Facilitate access to community resources, services, and supports, including housing, income support, and legal/guardianship matters.
  • Deliver education and support to families and caregivers regarding mental illness, aging, and system navigation.
  • Use evidence-informed practices to guide interventions and advocate for appropriate care and resources.
  • Document and maintain accurate records in compliance with professional and organizational standards.
  • Participate in quality improvement initiatives, interdisciplinary meetings, and ongoing professional development.
  • Comply with legislation, organizational policies, and best practices related to privacy, ethics, safety, and occupational health.
  • Acts as a role model, mentor, and/or preceptor for learners, new grads, and new hires from various disciplines.

Qualifications

  • Bachelor’s Degree in Social Work from an accredited university.
  • Registration in good standing with the College of Social Workers and Social Service Workers (OCSWSSW).
  • Valid Ontario driver’s license and a reliable vehicle required, as the role involves travel within the community.

Experience

  • Minimum 3 years of experience in mental health, with a focus on older adults.

Skill/Ability

  • In-depth understanding of mental health conditions in older adults and the psychosocial impact of aging.
  • Strong assessment skills, including the ability to evaluate cognitive, emotional, and social functioning.
  • Knowledge of the Mental Health Act, Substitute Decisions Act and relevant provincial legislation.
  • Familiarity with psychotropic medications and their social implications.
  • Ability to navigate and coordinate complex systems of care and community supports.
  • Demonstrated advocacy skills and cultural competence in working with diverse populations.
  • Excellent communication, organizational, and documentation skills.
  • Ability to work independently collaboratively within an interdisciplinary team.
  • Proficiency in Microsoft Office applications (Word, Excel, Outlook) and electronic health record systems.
