The Registered Practical Nurse (RPN) reports to the Director of Care and is expected to coordinate and oversee all aspects of resident-focused care delivery. An RPN should also promote the values, goals, and philosophy of the Home.
Responsibilities:
- Develop and modify care plans based on residents’ individual health needs and changes in health status.
- Liaise with healthcare personnel, agencies, and families.
- Maintain good relationships with team members, residents, families, and the public.
- Engage in meaningful interactions with residents during shifts.
- Supervise activities on the unit and take charge as required.
- Monitor performance and complete evaluations for PSWs.
- Direct, assign, and evaluate PSWs’ work.
- May be in charge of the Home as needed, utilizing leadership skills to identify and solve problems.
- Ensure care delivery aligns with the resident Bill of Rights.
- Understand and participate in quality indicator improvement efforts.
- Address resident and family concerns appropriately.
- Transcribe and process physician's orders in accordance with CNO standards.
- Manage nursing supplies and equipment efficiently.
- Conduct unit rounds at shift start and end.
- Administer medications and treatments per policies and document all procedures accurately.
- Maintain up-to-date clinical assessments, care plans, and documentation.
- Participate in resident care conferences.
- Keep accurate records of nursing observations and care.
- Assess health status changes and communicate with SDMs and the interdisciplinary team.
- Complete assessments and develop/update care plans collaboratively.
- Motivate and empower team members.
- Perform other assigned tasks as needed.
Qualifications:
- Current Certificate of Competence with the College of Nurses of Ontario or equivalent registration in another Canadian jurisdiction.
- Basic CPR certification is an asset.
- Experience in long-term care is an asset.
- Ability to work well with interdisciplinary teams.
- Strong communication skills.
- Knowledge of RAI-MDS; AIS certification preferred, or must be completed within one year of hire.
- Clear vulnerable sector screen (less than 6 months old), recent TB test, and three supervisory references.
